When you book in as a client:
I require information in order to provide you with the best quality comprehensive care. This clinic (Raelene Callaway trading as “The Village Naturopath” follows the guidelines of best practice for the management of health information in private practice. This means your personal health information is kept private and secure as required by federal and state privacy laws as well as per the guidelines set out by the Naturopaths and Herbalists Association of Australia (NHAA), of which I am a member.
To ensure your privacy, this practice adheres to the National Privacy Principles of the Council of Australian Privacy Legislation. Your health information is collected by us only with your consent and as necessary for the proper and effective treatment of your condition. Health information about you will not be released to any other party without your consent.
You may review your health information with your treating provider and are entitled to access your health records for this purpose. If you have any queries about the confidentiality of your health information please feel welcome to discuss this with your health provider.
To provide a high standard of complementary and alternative health care I need to collect personal information from my clients. I only collect, record and store information that is pertinent to the case. I may be legally bound to disclose personal information to authorities in the case of mandatory reporting I will take all reasonable steps to ensure that personal information is accurate, complete and up to date, and I request your compliance in this matter. Please advise me if your details have changed.

As my client you agree to the following:
1. You fully understand that Raelene Callaway is not a medical doctor or practitioner and you are not seeking medical diagnostic or treatment procedures.
2. The services performed by Raelene Callaway are at all times restricted to consultation on the subject of exercise, nutritional and naturopathic matters intended for the maintenance of the best possible state of holistic health, and do not involve the diagnosing, prognostication, treatment or prescribing of remedies for the treatment of disease, or any act which will constitute the practice of medicine in the state of Tasmania, for which a license is required.
